ProVital Transit (“ProVital Transit,” “we,” “us,” or “our”) provides non-emergency medical transportation (NEMT) in Charlotte, North Carolina and the surrounding communities. This policy applies to information collected through this website and our related online forms and communications. By using this site or submitting a form, you agree to the practices described here. If you do not agree, please do not use the site.

When you submit our ride-request or contact form, request a callback, or otherwise contact us, we collect the information you choose to give us, which may include your name, phone number, email address, preferred contact method, preferred times, the general type of service you’re interested in, the city or area you need service in, and any message you include.

Our online forms are not designed to collect medical, health, or diagnosis information. Please do not enter detailed medical information into our forms. Trip details that involve your health or mobility needs are gathered securely by phone with a coordinator.

Information collected automatically

When you visit the site, we (and our service providers) automatically collect limited technical information such as your browser type and version, device and operating system, referring page, the pages you view, and the date and time of your visit. For security and abuse prevention we may store a one-way hashed (not reversible) version of your IP address rather than the address itself.

3. How We Use Your Information

We use the information described above to:

  • Respond to your request and arrange or coordinate transportation.
  • Contact you about your ride, inquiry, or account, including by phone, text, or email.
  • Confirm trip details, scheduling, eligibility, and any accessibility needs.
  • Operate, maintain, secure, and improve the website and our services.
  • Detect, prevent, and respond to fraud, abuse, and security incidents.
  • Comply with our legal obligations and enforce our terms.

We do not use your information for automated decision-making that produces legal or similarly significant effects, and we do not sell your personal information.

4. How We Share Information

We do not sell or rent your personal information. We share it only in the limited circumstances below:

  • Service providers. With vendors who perform services on our behalf — for example, website hosting, email delivery, secure cloud storage, and analytics — under contracts that limit their use of your information to providing those services.
  • Coordination of your transportation. Where you ask us to, we may share necessary details with the parties involved in arranging your trip (such as a facility, caregiver, or the broker/health plan handling your benefit).
  • Legal and safety. When required by law, subpoena, or government request, or when necessary to protect the rights, safety, or property of ProVital Transit, our riders, or the public.
  • Business transfers. In connection with a merger, acquisition, financing, or sale of assets, your information may be transferred as part of that transaction, subject to this policy.

5. Cookies & Tracking

Cookies are small text files stored on your device. We use only essential and functionality cookies needed to run the site, keep it secure, and remember basic preferences. We do not use cookies to build advertising profiles or to sell your data.

6. Health Information

ProVital Transit takes the sensitivity of health information seriously. Our website is a general marketing and intake tool and is not intended to receive protected health information (PHI). Information about your condition, treatment, or trip is collected by phone with a coordinator and handled with appropriate care.

Where we provide transportation in connection with a covered health plan or a HIPAA-covered entity and act as a business associate, we handle any protected health information in accordance with our agreements and applicable law, including HIPAA where it applies.

7. Data Retention

We keep personal information only for as long as needed for the purposes described in this policy — for example, to coordinate your transportation, maintain records of our communications, meet legal, tax, and recordkeeping obligations, and resolve disputes. When information is no longer needed, we take reasonable steps to delete it or de-identify it.

8. How We Protect Your Information

We use administrative, technical, and physical safeguards designed to protect your information against unauthorized access, loss, misuse, and alteration — including encrypted connections (HTTPS), access controls, and reputable hosting and email providers. No method of transmission or storage is completely secure, however, and we cannot guarantee absolute security. Please don’t send sensitive medical details through our web forms or by email.

9. Your Privacy Rights & Choices

Depending on where you live, you may have rights regarding your personal information, including the right to:

  • Access or receive a copy of the personal information we hold about you.
  • Correct inaccurate information.
  • Request deletion of your information.
  • Opt out of certain processing, and direct us not to sell or share your information (we do not sell it).

You may also unsubscribe from non-essential emails using the link in those messages, or ask us to stop contacting you for marketing purposes at any time.

10. Children's Privacy

Our website is intended for adults arranging transportation and is not directed to children under 13. We do not knowingly collect personal information from children under 13 through the site. If you believe a child has provided us information through the website, please contact us and we will delete it.

We may update this Privacy Policy from time to time to reflect changes in our practices, the services we offer, or the law. When we do, we will revise the “Last updated” date at the top of this page. Material changes will be made clear on this page. Your continued use of the site after an update means you accept the revised policy.
